While antibiotics remain the primary etiotropic treatment, their efficacy is increasingly compromised by the emergence of microbial resistance, significant risks of severe adverse reactions, and numerous contraindications associated with certain pharmacological agents. These challenges have catalyzed the development of novel formulations that offer improved safety profiles and enhanced efficacy. This review presents findings on the use of doxycycline (tetracycline class) and josamycin (macrolide class) in the management of pyoderma and vulgar acne. Doxycycline is characterized by its broad-spectrum bacteriostatic action and long-lasting anti-inflammatory effect, which facilitate its widespread use in treating acne and other infectious skin disorders, including boils, carbuncles, abscesses, and erysipelas. The contemporary dispersible formulation of doxycycline monohydrate is noted for its ease of administration, rapid absorption, ability to achieve high concentrations at infection sites, and reduced local gastrointestinal irritation. Josamycin attains high levels within inflammatory foci, exhibits excellent permeability across histohematological barriers, and demonstrates bacteriostatic and post-antibiotic effects, characterized by prolonged maintenance of high drug levels at the site of inflammation. The attributes of the available tablet formulation of josamycin include minimal impact on the normal intestinal microflora, suitability for use during pregnancy, and favorable tolerability. The evidence presented suggests that both doxycycline and josamycin are highly effective in the treatment of various infectious skin diseases with a low incidence of adverse drug reactions. Therefore, these agents are preferred options for systemic antibiotic therapy in pyoderma and acne, especially when alternative antibacterial agents are poorly tolerated or have low efficacy.</p></abstract><trans-abstract xml:lang="ru"><p>В настоящее время антибактериальная терапия играет важную роль в лечении инфекционных заболеваний кожи и ее придатков, включая пиодермии и пустулезную форму акне. Несмотря на то, что антибиотики остаются ключевым этиотропным методом лечения, их эффективность ограничивается растущей резистентностью микроорганизмов, значительным риском развития тяжелых побочных реакций и большим количеством противопоказаний к тому или иному препарату. Эти особенности способствуют разработке и появлению новых лекарственных форм препаратов, отличающихся более благоприятным профилем безопасности и эффективностью. В обзоре представлены данные о применении антибиотика тетрациклинового ряда доксициклина и джозамицина, относящегося к группе макролидов, в лечении пиодермий и вульгарных акне. Доксициклин обладает широким спектром бактериостатического действия и продолжительным противовоспалительным эффектом, благодаря чему он нашел широкое применение в лечении акне и ряда других инфекционных заболеваний кожи, включая фурункулы, карбункулы, абсцесс и рожистое воспаление кожи. Современная диспергируемая форма доксициклина моногидрата обладает удобством в применении, обеспечивает быстрое всасывание, высокую концентрацию в очаге инфекции и сниженное местное раздражающее действие на слизистую оболочку органов желудочно-кишечного тракта. Джозамицин, в свою очередь, создает высокие концентрации в очаге воспаления, обладает высокой проходимостью через гистогематические барьеры, бактериостатическим и постантибиотическим эффектом, что проявляется в виде сохранения высоких концентраций препарата в очаге воспаления в течение длительного времени. Особенностями представленной таблетированной формы джозамицина являются минимальное действие на нормальную микрофлору кишечника, возможность использования препарата при беременности и хорошая переносимость. Приведенные данные свидетельствуют о том, что оба препарата обладают высокой эффективностью в лечении большинства инфекционных заболеваний кожи и низкой частотой развития нежелательных лекарственных явлений.
