However, despite repeated high-level statements, global public health efforts have been limited to prioritizing cardiovascular diseases, cancer, respiratory diseases, and diabetes. The World Health Organization's decision to submit a resolution to the World Health Assembly in May 2026 to include fatty liver disease in the global definition of NCDs is a welcome change of course. Metabolically associated fatty liver disease (MAFLD) is of significant diseases, associated with adverse cardiovascular events, a high risk of developing chronic kidney disease, and extrahepatic malignancies. However, despite its high prevalence and potential for serious complications, therapeutic options for treating MAFLD and associated conditions remain limited. Current treatment strategies, primarily based on lifestyle modification and aimed at weight loss, despite demonstrated high efficacy, do not always achieve the ultimate goal. Maintaining clinically significant weight loss remains challenging. Medications with hepatotropic effects registered in the Russian Federation for the treatment of MAFLD have proven effective in combination therapy, primarily for the treatment of hepatic steatosis. In recent years, medications marketed for the treatment of obesity, such as glucagon-like peptide-1 receptor agonists (GLP-1RA), have attracted considerable attention from the medical community due to their significant therapeutic efficacy and promising pleiotropic effects. The approval of semaglutide, a GLP-1RA drug, for the treatment of metabolically associated steatohepatitis was an important milestone in the treatment of this patient group. The review article presents data on the effectiveness of treatment with drugs belonging to the GLP-1RA in the treatment of MAFLD, combined comorbidity, as well as possible combination with hepatoprotective drugs traditionally used in the treatment of MAFLD.</p> <p> </p></abstract><trans-abstract xml:lang="ru"><p>Снижение заболеваемости и смертности от неинфекционных заболеваний (НИЗ) является одной из важнейших задач здравоохранения XXI в. Однако, несмотря на неоднократные заявления на высоком уровне, глобальные меры в области общественного здравоохранения были ограничены приоритетом сердечно-сосудистых заболеваний, рака, респираторных заболеваний и сахарного диабета. Решение Всемирной организации здравоохранения внести на рассмотрение Всемирной ассамблеи здравоохранения в мае 2026 г. резолюцию о включении стеатотической болезни печени в глобальное определение НИЗ является долгожданным изменением курса. Метаболически ассоциированная жировая болезнь печени (МАЖБП) имеет важное клиническое значение, поскольку она связана с неблагоприятными сердечно-сосудистыми событиями, высоким риском развития хронической болезни почек и внепеченочных злокачественных новообразований. Между тем, несмотря на высокую распространенность и потенциал серьезных осложнений, терапевтические возможности для лечения МАЖБП и ассоциированных с ней заболеваний остаются ограниченными. Современные стратегии лечения, которые базируются прежде всего на изменении образа жизни, направленные на снижение массы тела пациента, несмотря на продемонстрированную высокую эффективность, не всегда достигают конечной цели. Это связано с тем, что поддержание клинически значимой потери массы тела остается сложной задачей. Препараты с гепатотропными эффектами, зарегистрированные в Российской Федерации для лечения МАЖБП, хорошо зарекомендовали себя для использования в составе комбинированной терапии, в первую очередь для лечения стеатоза печени. В последние годы препараты, которые позиционированы для лечения ожирения, такие как агонисты рецепторов глюкагоноподобного пептида-1 (арГПП-1), стали предметом пристального внимания медицинского сообщества благодаря значительной терапевтической эффективности и широким перспективам их плейотропного действия. Регистрация препарата из группы арГПП-1 семаглутида для лечения метаболически ассоциированного стеатогепатита стала важной вехой в лечении этой категории пациентов.
