Как подчеркивается в этих руководствах, основной задачей медикаментозной терапии является установление длительного контроля над заболеванием, а не просто купирование симптомов.Новым способом достижения контролируемого течения БА является концепция лечения единым ингалятором (стратегия SMART – Symbicort Maintenance And Relief Treatment), или ранее известный как SIT (Single Inhaler Therapy – терапия при помощи одного ингалятора). Важной особенностью стратегии SMART является способность Симбикорта (160/4,5) быть не только базисным препаратом для контроля воспалительного процесса, но и немедленно облегчать симптомы астмы. Таким образом, в момент затрудненного дыхания пациент не только применяет бронхорасширяющее средство, но и противовоспалительный препарат – будесонид.Из всех ИГКС будесонид имеет наиболее благоприятный терапевтический индекс, что связано с его высоким сродством к глюкокортикоидным рецепторам и ускоренным метаболизмом после системной абсорбции в легких и кишечнике.
