Болезнь Крона (БК) и язвенный колит (ЯК) относятся к большой гетерогенной группе воспалительных заболеваний кишечника (ВЗК). Их дифференциальная диагностика может быть затруднена из-за нехватки единственного отличительного признака, так называемого золотого стандарта. Проблемы возникают наиболее остро тогда, когда воспаление ограничено толстой кишкой, где проявления этих двух заболеваний частично совпадают (перекрываются).Даже в опытных центрах точный диагноз ЯК и БК не может быть установлен приблизительно в 5–10% случаев при заболевании, ограниченном толстой кишкой. В этих случаях клиницисты (чаще) и патоморфологи (реже) обычно используют термин неопределенный колит. Таким образом, пациенты с диагнозом "неопределенный колит" могут представлять собой гетерогенную группу. Некоторые из них страдают колитом, который, несмотря на исследования, проводимые гастроэнтерологами, эндоскопистами, патоморфологами и хирургами, не может быть отнесен к ЯК или БК (поэтому называется неопределенным колитом). Другие, которым диагноз "неопределенный колит" был поставлен в менее специализированных центрах, могут вполне иметь ЯК или БК, если исследования будут проведены более опытными специалистами.
