Однако несмотря на многообразие антидиабетических средств и возможность их комбинированного применения, достижение компенсации метаболических нарушений у достаточно большой части больных СД типа 2 остается труднодостижимой задачей. Результаты многочисленных исследований подтверждают, что более 60% пациентов имеют неудовлетворительную компенсацию СД типа 2 [1, 2]. Согласно данным исследования UKPDS эффективность терапии по мере увеличения длительности СД типа 2 снижается: целевых показателей HbA1c&lt;7% достигают около 50% пациентов в течение 3 лет и лишь 25% пациентов – при длительности СД 9 лет [3]. Основной причиной ухудшения компенсации с течением времени является прогрессирующее снижение секреторной функции b-клеток (рис. 1). К сожалению, назначение пациентам c СД типа 2 экзогенного инсулина часто не приводит к улучшению показателей гликемии. Так, оптимальный контроль диабета (HbA1c&lt;7%) имеют 13–30% пациентов при терапии различными пероральными сахароснижающими препаратами (ПССП) и такое же число пациентов (26%) при комбинации ПССП с инсулином [4]. Таким образом, интенсивная терапия СД типа 2, требующая назначения больному большого количества лекарственных препаратов, не всегда сопровождается соответствующим повышением эффективности и в то же время сопряжена с увеличением риска побочных эффектов (гипогликемия, увеличение массы тела) и низкой приверженностью пациента лечению. В этой связи появление нового класса препаратов с отличным от всех ранее известных механизмов действия – миметиков инкретина – представляет несомненный клинический интерес.
