В течение второй половины ХХ в. РААС стала объектом фармакологических воздействий, преимущественно блокирующих ее на разных уровнях и в связи с этим позволявших добиться снижения повышенного АД, а также затормозить процессы дезадаптивного ремоделирования органов-мишеней (миокард, сосудистая стенка, почка). Ингибиторы ангиотензинпревращающего фермента (ИАПФ) и блокаторы рецепторов ангиотензина II сегодня – принципиально важный компонент стратегии длительного ведения больных АГ высокого и очень высокого риска, а также сахарного диабета типа 2, хронической сердечной недостаточности и хронических заболеваний почек с протеинурией [1]. Диапазон применения антагонистов альдостерона несколько уже – их используют для лечения хронической сердечной недостаточности [2] и особых вариантов АГ, в частности, возникающей при первичном гиперальдостеронизме [3], а также не уступающей стандартным комбинациям антигипертензивных препаратов [4]. В настоящее время, спустя 110 лет после открытия ренина, можно утверждать, что прямая блокада его эффектов приобрела статус самостоятельного подхода к антигипертензивной терапии, имеющей ряд свойств, не характерных для препаратов, блокирующих РААС на других уровнях.</p></abstract></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><mixed-citation>Комитет экспертов Российского медицинского общества по артериальной гипертонии и Всероссийского научного общества кардиологов.
