Необходимость назначения нескольких гипотензивных препаратов для достижения целевого уровня АД основывается на данных крупных многоцентровых клинических исследований.Комбинированная терапия как приоритетное направление в лечении АГ.В новых рекомендациях Российского медицинского общества артериальной гипертонии/Всероссийского научного общества кардиологов (РМОАГ/ВНОК) назначение комбинации двух гипотензивных препаратов рассматривается как альтернатива монотерапии уже в начале лечения [2]. Используются комбинации из двух, трех и более антигипертензивных препаратов. Комбинированная терапия имеет много преимуществ: усиление антигипертензивного эффекта за счет разнонаправленного действия препаратов на патогенетические механизмы развития АГ, что увеличивает число пациентов со стабильным снижением АД; уменьшение частоты возникновения побочных эффектов как за счет меньших доз комбинируемых средств, так и за счет взаимной нейтрализации этих эффектов; обеспечение наиболее эффективной органопротекции и уменьшение риска и числа сердечно-сосудистых осложнений.Комбинация препаратов, блокирующих активность ренин-ангиотензин-альдостероновой системы (РААС) – ИАПФ, БРА, и диуретиков является наиболее популярной в терапии АГ благодаря высокой АД-снижающей эффективности, защите органов-мишеней, хорошей безопасности и переносимости, а также привлекательным фармакоэкономическим показателям.В целом ряде клинических исследований было показано, что одновременное назначение АК и препаратов, блокирующих активность РААС (ИАПФ или БРА), вызывает большее снижение АД, чем компоненты по отдельности [5, 10, 11].
