В основе постоянно протекающих процессов костной перестройки лежит активность двух типов костных клеток – остеобластов и остеокластов, отвечающих, соответственно, за синтез костного матрикса и костную резорбцию. Дисбаланс между процессами резорбции и образования костной ткани лежит в основе патогенеза остеопороза. У женщин в первые 10 лет после менопаузы отмечается ускоренное разрушение костной ткани, и за этот период они могут потерять до 30% костной массы [1]. Возможности терапии остеопороза Идеальный антиостеопорозный препарат должен быть способным повышать активность образования костной ткани и подавлять костную резорбцию, что позволит восстановить нормальный процесс перестройки костной ткани. Стронция ранелат В этом плане заслуживает особого внимания препарат стронция ранелат. Молекула стронция ранелата (Бивалос) состоит из двух атомов стабильного, нерадиоактивного стронция и органического основания – ранеловой кислоты. Химическое название препарата: дистронциевая соль 5-[би-(карбоксиметил)амино]-2-карбокси-4-циано-3-тиофен-уксусной кислоты. Относительная молекулярная масса составляет 513,49 г/моль, содержание стронция ранелата в препарате – 34,1% [2]. Большинство применяемых в настоящее время антиостеопорозных препаратов подавляет костную резорбцию (бисфосфонаты, селективные модуляторы рецепторов эстрогена и кальцитонин), что приводит к общему снижению активности костного метаболизма. Экспериментальные и клинические исследования свидетельствуют о том, что стронция ранелат обладает двойным механизмом действия на обмен костной ткани. Препарат увеличивает активность синтеза костной ткани и уменьшает костную резорбцию, что приводит к восстановлению баланса костного метаболизма в пользу образования новой костной ткани.
