У препаратов этой группы есть несколько общих хорошо известных эффектов – анальгетический, противовоспалительный и жаропонижающий, благодаря которым они пользуются популярностью у врачей и пациентов. В основном эти эффекты реализуются за счет ингибиции фермента циклооксигеназы (ЦОГ) 2-го типа, который отвечает за выработку провоспалительных простагландинов и других медиаторов воспаления. Нежелательные эффекты связаны с подавлением фермента ЦОГ-1, снижением синтеза цитопротективных простагландинов. Этот факт дал возможность синтезировать новый класс потенциально более безопасных НПВП – ЦОГ-2-селективных, что открыло ряд новых терапевтических возможностей. Нимесулид Среди ЦОГ-2-селективных НПВП наиболее популярным и широко используемым препаратом на сегодняшний день является нимесулид, имеющий хорошую переносимость, высокую безопасность, большой выбор лекарственных форм. Он применяется в 50 странах мира под различными торговыми названиями с 1985 г. В России в 2007 г. было продано более 13 млн упаковок нимесулида. В Италии он назначается врачами чаще, чем другие препараты этой группы. Препарат был разработан швейцарской фирмой Helsinn Healthcare в 1980 г. В 1994 г. показана способность нимесулида ингибировать ЦОГ-2 при значительно меньшем влиянии на ЦОГ-1, которая в дальнейшем была неоднократно подтверждена в исследованиях in vitro, ex vivo, а также in vivo. В данном случае умеренная селективность может являться преимуществом по сравнению с высокой селективностью. Предполагается, что нимесулид ингибирует ЦОГ-1 именно в очаге воспаления и боли, что может носить особое терапевтическое преимущество при отсутствии влияния на ЦОГ-1 в желудке и почках.
