Это осложнение развивается у 40–50% больных СД типа 1 и 2 при длительности заболевания более 20 лет. В США по данным регистра больных, получающих заместительную почечную терапию (ЗПТ), СД является самой частой причиной развития хронической почечной недостаточности (ХПН), составляя почти 44% [1]. В России по данным аналогичного регистра больных с ХПН доля больных СД в структуре диализной службы не превышает 9,5% [2] (рис. 1).В то же время истинная потребность в ЗПТ у больных СД в РФ по расчетам, полученным в ходе эпидемиологических исследований, проведенных ФГБУ Эндокринологический научный центр Минздравсоцразвития РФ, не уступает таковой в развитых странах мира. Согласно этим данным фактическая распространенность диабетической нефропатии (ДН) в регионах России в среднем составляет 43% [3].Самым важным вопросом, волнующим каждого клинициста, является возможность регрессии и ремиссии ДН. Под регрессией ДН следует понимать улучшение показателей работы почек, а под ремиссией – их полную нормализацию. При этом следует различать лабораторную и морфологическую (структурную) регрессию или ремиссию ДН.Ряд долгосрочных исследований, проведенных у больных СД типа 1 и 2, позволяют утверждать, что МАУ является обратимой стадией поражения почек при СД. Так, в исследованиях EURODIAB [5], STENO-2 [6], S.Araki [7] и Kashiwa study [8] оценивали влияние длительного многофакторного интенсивного лечения больных СД как на частоту прогрессирования нефропатии от стадии МАУ к стадии ПУ, так и на частоту ремиссии ДН от МАУ к нормоальбуминурии (НАУ). Как следует из представленных данных, одновременный интенсивный контроль гликемии и артериально- го давления (АД) значимо чаще приводил к нормализации уровня альбуминурии, чем к ее прогрессированию до степени протеинурии.В исследовании STENO-2 [6] статистически оценены относительные риски (ОР) прогрессирования и обратного развития ДН.</p></abstract></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><mixed-citation>Excerpts from the United States Renal Data System 2007 annual data report.
