Так, результаты относительно недавно выполненных исследований свидетельствовали о том, что смертность в течение 1 мес после развития ОКС составляет 4,6%, а в ходе выполнения крупного исследования TRITON-TIMI 38 (Trial to assess Improvement in Therapeutic Outcomes by optimizing platelet inhibitioN with prasugre l -Thrombolysis in Myocardial Infarction 38) смертность в течение 15 мес наблюдения при использовании современной тактики лечения составляла лишь 3,1%.Результаты рандомизированных клинических испытаний свидетельствовали о том, что применение ацетилсалициловой кислоты (АСК), b-блокаторов, блокаторов ренин - ангиотензин - альдостероновой системы и тиенопиридиновых антиагрегантов приводит к статистически значимому улучшению прогноза больных, перенесших ОКС. Однако частота использования таких препаратов в реальной клинической практике отличается от оптимальной. Результаты анализа данных регистра SAMI (Secondary prevention after Acute Myocardial Infarction) свидетельствуют о том, что при выписке из стационара больных, перенесших ОКС, частота назначения АСК, b-блокаторов, статинов, блокаторов РААС и тиенопиридиновых антиагрегантов составляет 89, 90, 84, 81 и 70% соответственно. Через 1 год после выписки частота применения препаратов, относящихся к указанным классам, снижалась.В реальной клинической практике (в отличие от результатов клинических исследований) в течение года частота развития повторного инфаркта миокарда (ИМ) достигает 11%, а смертность превышает таковую в общей популяции в 2 раза в течение первых 30 дней (относительный риск – ОР 2,1 доверительный интервал – ДИ 95% 1,9–2,5) и более чем в 5 раз в последующем (ОР 5,6,95% ДИ 5,1–6,2). Таким образом, в реальной клинической практике соблюдение предписанного режима терапии имеет даже большее значение, поскольку риск развития осложнений сердечно - сосудистых заболеваний в таких условиях выше, чем в ходе выполнения специально организованных клинических испытаний.</p></abstract></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><mixed-citation>Assessment of the Safety and Efficacy of a New Treatment Strategy with Percutaneous Coronary Intervention (ASSENT-4 PCI) investigators.
