В числе основных факторов сердечно-сосудистого риска наряду с дислипидемией, сахарным диабетом (СД), курением, ожирением находится артериальная гипертония (АГ).Очевидно, что выбранный антигипертензивный препарат должен не только снижать артериальное давление (АД), но и благоприятно влиять на липидный и углеводный виды обмена или как минимум не ухудшать состояние имеющихся факторов риска, а также обеспечивать регресс поражения органов-мишеней.В настоящее время как зарубежные, так и отечественные Рекомендации по диагностике и лечению АГ (Российское медицинское общество по АГ и Всероссийскоенаучное общество кардиологов – ВНОК) [1] предлагают использовать 5 основных классов антигипертензивных препаратов, однако приоритеты изменились: если в течение многих лет препаратами выбора оставались b-блокаторы и диуретики, то сегодня на первый план вышли ингибиторы ангиотензинпревращающего фермента (ИАПФ) и блокаторы рецепторов ангиотензина II (БРА), также известные как сартаны. Это связано с тем, что ренин-ангиотензин-альдостероновая система (РААС) играет центральную роль как в возникновении АГ, так и в развитии сопутствующих ей серьезных сердечно-сосудистых осложнений, таких как мозговой инсульт, инфаркт миокарда (ИМ), нефроангиосклероз, застойная сердечная недостаточность и др.
