Популяризация скрининговых программ привела к«омоложению» РПЖ за счет диагностирования заболевания у мужчин молодого возраста и выявления ранних форм заболевания. Данный факт позволил предлагать пациентам различные методы лечения локализованных форм РПЖ, в частности РПЭ. Позадилонная РПЭ из открытого доступа (ОРП), предложенная T.Millin в 1945 г. и модифицированная P.Walsh и соавт. в 1980 г., на протяжении длительного времени являлась«золотым стандартом» лечения клинически локализованных форм РПЖ. За последние десятилетия благодаря прогрессивному развитию минимально инвазивных технологий в медицине взгляды урологов изменились в пользу применения робот-ассистированной техники выполнения РПЭ. В настоящее время роботизированные технологии занимают ведущие позиции в хирургии и урологии,превосходя уже широко использующиеся лапароскопические методики. РАРПЭ - наиболее популярная роботическая операция во всем мире. Роботическая РПЭ становится «золотым стандартом» лечения локализованного РПЖ, вытесняя открытую операцию. Многочисленные работы по изучению и анализу выполнения РАРПЭ позволяют выделить преимущества роботической хирургии с точки зрения доказательной медицины.В настоящее время в клинике урологии МГМСУ накоплен наибольший в России опыт хирургического лечения РПЖ. Следует отметить, что кроме лечения рака простаты роботическая техника используется в лечении опухолей почек, мочевого пузыря, мочекаменной болезни, а также при пластических операциях на мочеточниках и при пролапсе гениталий.В период с ноября 2008 по июнь 2014 г. в клинике урологии МГМСУ РПЭ с использованием роботической установки da Vinci S/Si выполнена 975 пациентам.
