Abstract

Modern views on the diagnosis of idiopathic interstitial pneumonia (IIP) are based on multidisciplinary approach. The paper considers main types of IIP, as well as the principles of their differential diagnosis. Idiopathic pulmonary fibrosis histologically referred to as usual interstitial pneumonia, in which diffuse fibrosis causes a restructuring of the parenchyma of the lungs.

Imaging of pulmonary nontuberculous mycobacterial infection

Sokolina I.A.

Abstract

The nontuberculous mycobacterial infection (NTMB) usually cause chronic, indolent pulmonary infection. The purpose of this article is to review radiologic manifestations of pulmonary nontuberculous mycobacterial infection. Common and well-recognized patterns of NTMB infection include cavitary and bronchiectatic disease. Less common manifestations include nodules or masses mimicking malignancy and others. Definitive diagnosis can be difficult and patterns may overlap. Knowledge of the full spectrum of radiological findings is important to facilitate diagnosis and treatment.

Differential diagnosis of cavitary and destructive processes in the lung

Koroleva I.M.

Abstract

The variety of cavitary and destructive processes in the lungs is determined by a variety of etiological and pathogenetic factors causing these conditions. A number of states are the result of exposure to a pathological agent, and the other part of cavitary processes in the lungs caused by a complex genetic mechanism. Of special interest are destructive processes in the lungs caused by rare pathogenic agents against the backdrop of severe diseases.

Stepanova E.A., Vishnyakova M.V., Sambulov V.I., Podrez D.V.

Abstract

We showed the clinical observation of rarely seen Gradenigo’s syndrome in 4-year-old child. The results of complex radiologic imaging findings of diseases of each patient (multislice computed tomography, magnetic resonance imaging) were showed in the observation and these results allowed to make differential diagnosis with neoplasms and to determine the therapeutic approach.

Abstract

The paper discusses major clinical signs, and treatment approaches allergic rhinitis (AR). The importance of intranasal steroids as basic medications. Mometasone furoate is superior to other topical steroids since it is characterized by the broadest spectrum of approved indications for use, high efficacy, very low bioavailability, and long-term experience of clinical use. A novel combination of mometasone furoate, intranasal steroid, and azelastine, topical antihistamine, for mild and moderate intermittent AR is presented. This nasal spray is the first and the only medication in Russia which combines the most effective members of their classes with different mechanism of action.

The characteristics of diagnosis of system vasculitis in the respiratory practice

Zaytseva A.S., Shmelev E.I., Stepanyan I.E., Shabalina I.Y., Dmitrieva L.I., Evgushchenko G.V.

Abstract

Pulmonary vasculitis is a relatively rare disease and is often a source of diagnostic errors. Pulmonary involvement, associated with the systemic vasculitis, can be the primary or even only one manifestation of systemic diseases, and phthisiologists and pulmonologists first face with pulmonary vasculitis in medical practice. The article deals with the clinical manifestations of most often disorder such as ANCA-associated necrotizing vasculitis which is also called granulomatosis with polyangiitis. This article presents, in the form of a clinical illustration, the difficulties of this disease diagnosis.

The role of electrical myostimulation rehabilitation programme in patients with chronic obstructive pulmonary disease (clinical examples)

Meshcheriakova N.N., Kunafina T.V., Belevskii A.S.

Abstract

Dysfunction of skeletal and respiratory muscles is one of the most common systemic effects in patients with chronic obstructive pulmonary disease (COPD). Currently, only the methods of pulmonary reabilitation, such as physical exercise, can change the functional status of patients, to improve physical activity through the improvement of skeletal and respiratory muscles. The use of neuromuscular electrical stimulation is an important rehabilitation program in COPD patients with severe physical limitations. This is confirmed by the results of studies and clinical examples.

Abstract

Ventilator-associated pneumonia (VAP), is the most common infection in critically ill patients, significantly increases the risk of death, often caused by resistant flora (Pseudomonas aeruginosa, Acinetobacter baummannii) and requires significant material costs associated with the use of antibiotics. Published guidelines that define the basic issues related to diagnosis, treatment and prevention VAP. Widely discussed various aspects of antibacterial therapy VAP that is associated with the emergence of new antimicrobial agents, complexity, urgency of the problem at the interface of fundamental and applied science, ethical, humanitarian, and clinical interests.

Trukhan D.I., Bagisheva N.V.

Abstract

Bronchial asthma is a common chronic diseases. The article considers the role and place of the doctor of first contact (physicians and general practitioners) in the diagnosis and treatment of bronchial asthma. The given clinical example demonstrates the complexity of diagnosing bronchial asthma of mild severity combined with allergic rhinitis at the stage of primary health care, as well as the importance of prescribing complex therapy for all available allergic conditions (bronchial asthma + allergic rhinitis) to achieve a positive result.

The characteristics of pathogenesis and therapy of acute rhinosinusitis in patients with allergic rhinitis

Kirdeeva A.I., Kosiakov S.Y.

Abstract

The relationships between acute rhinosinusitis and allergic rhinitis are not fully understood, and therefore the question concerning the characteristics of ARS therapy in patients with concomitant allergy remains open. Multiple pathomechanisms including aspects of the immune response, specific cytokine profile, molecular mechanisms at the level of epithelial cells and intercellular matrix are common for allergic rhinitis and rhinosinusitis. The mechanism of TGCSs action is inhibition of the synthesis of proinflammatory cytokines and preventing the migration of inflammatory cells. The use of intranasal glucocorticosteroids to leveling allergic inflammation may stop the development and increase the effectiveness of acute rhinosinusitis therapy.

Bodnya O.S., Nenasheva N.M., Andrenova G.V., Sinyavkin D.O., Bodnya Y.V., Kubanova E.T.

Abstract

Allergic rhinitis plays leading role in the structure of allergic diseases. The second generation of antihistamines are frequently used to cure allergic rhinitis, including seasonal manifestations. In the following paper the comparative results of open, randomize clinical studies of efficiency, safety and influenceon quality of life of patients, suffered from seasonal allergic rhinitis, treated by original AGP II ebastine and generic AGP II cetirizine and loratadine are discussed. Its efficiency and safety providing control on mild nasal and non-nasal symptoms of seasonal rhinitis in mature patients have been demonstrated. It is noticed that the rate of onset of effect and its expression in relation to nasaland non-nasal symptoms were much efficient in patients, obtained ebastine 20 mg comparing with cetirizine 10 mg and loratadine 10 mg.

The use of free combination of inhaled glucocorticosteroid and long-acting β2-agonist in patients with bronchial asthma: clinical examples

Sebekina O.V.

Abstract

The goal of asthma treatment is to achieve good control of symptoms and maintain normal activity level of the patient, minimizing the risk of future exacerbations and side effects of treatment. In accordance with modern guidelines, starting with stage 3 (moderate bronchial asthma patients) is the preferred therapy combination drugs with a combination of low-dose inhaled corticosteroids and long-acting b2-agonists.A meta-analysis of several studies demonstrated that the use of free combination of budesonide and formoterol significantly: improve lung function; reduce the need for short-acting b2-agonists; to improve the quality of life; reduce the number of exacerbations requiring hospitalization, call the ambulance, or the use of systemic corticosteroids. The use of free combination of budesonide and formoterol was accompanied by a high level of compliance. The article describes the clinical cases of the use of free combination of budesonide and formoterol in patients with asthma.
