Interstitial lung lesions attract the attention of the physicians and radiation physicians around the world similarity of clinical symptoms and radiological manifestations, along with a variety of pathological variants of the disease. This explains the difficulties of clinical and radiological diagnosis and differentiation of this group of lung diseases. This article focuses on the idiopathic pulmonary fibrosis.

The nontuberculous mycobacterial infection (NTMB) usually cause chronic, indolent pulmonary infection. The purpose of this article is to review radiologic manifestations of pulmonary nontuberculous mycobacterial infection. Common and well-recognized patterns of NTMB infection include cavitary and bronchiectatic disease. Less common manifestations include nodules or masses mimicking malignancy and others. Definitive diagnosis can be difficult and patterns may overlap. Knowledge of the full spectrum of radiological findings is important to facilitate diagnosis and treatment.

We showed the clinical observation of rarely seen Gradenigo’s syndrome in 4-year-old child. The results of complex radiologic imaging findings of diseases of each patient (multislice computed tomography, magnetic resonance imaging) were showed in the observation and these results allowed to make differential diagnosis with neoplasms and to determine the therapeutic approach.

The paper discusses major clinical signs, and treatment approaches allergic rhinitis (AR). The importance of intranasal steroids as basic medications. Mometasone furoate is superior to other topical steroids since it is characterized by the broadest spectrum of approved indications for use, high efficacy, very low bioavailability, and long-term experience of clinical use. A novel combination of mometasone furoate, intranasal steroid, and azelastine, topical antihistamine, for mild and moderate intermittent AR is presented. This nasal spray is the first and the only medication in Russia which combines the most effective members of their classes with different mechanism of action.

This article deals with the overview concerning mucoactive drugs, most commonly used for the treatment of inflammatory diseases of the upper respiratory tract. The overview describes the effect of acetylcysteine on bacterial biofilms and the clinical efficacy of combination of acetylcysteine and antibacterial drug in eradication of bacterial pathogens. The authors show the clinical case of successful treatment of 8 years old child with the relapse of purulent polysinusitis, using combination of drugs and acetylcysteine.

The article deals with the clinical manifestations of most often disorder such as ANCA-associated necrotizing vasculitis which is also called granulomatosis with polyangiitis. This article presents, in the form of a clinical illustration, the difficulties of this disease diagnosis.

The characteristics of pathogenesis and therapy of acute rhinosinusitis in patients with allergic rhinitis

The relationships between acute rhinosinusitis and allergic rhinitis are not fully understood, and therefore the question concerning the characteristics of ARS therapy in patients with concomitant allergy remains open. Multiple pathomechanisms including aspects of the immune response, specific cytokine profile, molecular mechanisms at the level of epithelial cells and intercellular matrix are common for allergic rhinitis and rhinosinusitis. The mechanism of TGCSs action is inhibition of the synthesis of proinflammatory cytokines and preventing the migration of inflammatory cells. The use of intranasal glucocorticosteroids to leveling allergic inflammation may stop the development and increase the effectiveness of acute rhinosinusitis therapy.

Comparative efficacy of different antihistamines II generation in adult patients with seasonal allergic rhinitis

Allergic rhinitis plays leading role in the structure of allergic diseases. The second generation of antihistamines are frequently used to cure allergic rhinitis, including seasonal manifestations. In the following paper the comparative results of open, randomize clinical studies of efficiency, safety and influenceon quality of life of patients, suffered from seasonal allergic rhinitis, treated by original AGP II ebastine and generic AGP II cetirizine and loratadine are discussed. Its efficiency and safety providing control on mild nasal and non-nasal symptoms of seasonal rhinitis in mature patients have been demonstrated. It is noticed that the rate of onset of effect and its expression in relation to nasaland non-nasal symptoms were much efficient in patients, obtained ebastine 20 mg comparing with cetirizine 10 mg and loratadine 10 mg.

The use of free combination of inhaled glucocorticosteroid and long-acting β2-agonist in patients with bronchial asthma: clinical examples

The goal of asthma treatment is to achieve good control of symptoms and maintain normal activity level of the patient, minimizing the risk of future exacerbations and side effects of treatment. In accordance with modern guidelines, starting with stage 3 (moderate bronchial asthma patients) is the preferred therapy combination drugs with a combination of low-dose inhaled corticosteroids and long-acting b2-agonists.A meta-analysis of several studies demonstrated that the use of free combination of budesonide and formoterol significantly: improve lung function; reduce the need for short-acting b2-agonists; to improve the quality of life; reduce the number of exacerbations requiring hospitalization, call the ambulance, or the use of systemic corticosteroids. The use of free combination of budesonide and formoterol was accompanied by a high level of compliance. The article describes the clinical cases of the use of free combination of budesonide and formoterol in patients with asthma.
