Abstract

New technologies of modern medicine make it possible to improve not only the treatment but also patients monitoring under the conditions of health care facilities. The invention of Internet-based technologies brings medical care to the patient, in their homes. Remotely observation of patients requiring constant medical supervision becomes an integral part of the "doctor-patient" relationship. The compensation of carbohydrate metabolism in people with diabetes is based not only on medical knowledge, but also on the patient desire to achieve results. This article discusses the new approaches to control glycemia using the Accu-Chek Connect Online portal.

The role of glimepiride in combination with metformin in modern practice of using antihyperglycemic therapy for type 2 diabetes mellitus

Abstract

The article describes the role of sulfonylurea medications in modern recommendations concerning the treatment of patients with diabetes mellitus type 2. We showed the characteristics of mechanism of action and pharmacokinetics of glimepiride associated with positive differences in the efficacy of treatment, the frequency of hypoglycemic reactions, effects on weight, the frequency of side effects and complications of the treatment. We described the results of clinical studies concerning extrapancreatic effects of glimepiride (effect on the synthesis of glycogen, lipid metabolism, ischemic preconditioning, insulin resistance index). We studied the efficacy of glimepiride (Amaryl) in combination with the rapid-release metformin fixed combination of Amaryl and metformin (Amaryl M).

The physician’s and endocrinologist’s role in the prevention of diabetic foot syndrome in patients with diabetes mellitus

Abstract

The diabetic foot syndrome takes the special place among the lower limbs decoders in case of diabetes mellitus because of the high frequency of disabilities and enormous economic damage. Effective prevention of the syndrome consists of several stages: primary prevention of chronic complications of diabetes mellitus (angiopathy, polyneuropathy), prevention of foot ulcers in patients with high risk of this condition development (secondary prevention), as well as the early treatment of the ulcerous defects of feet at the pre-hospital stage preventing amputations (tertiary prevention). While the treatment of foot ulcers is carried out by specialists (surgeons, cabinets of diabetic foot), the first two stages are under the control of the therapeutic and endocrinology outpatient services. Primary prevention is to achieve and maintain the glycemic and lipid metabolism targets, body weight, blood pressure level, smoking cessation. We discussed several aspects of using drug therapy of polyneuropathy and angiopathy in order to prevent their progression. We indicated the main issues on feet care, shoe selection, the frequency of regular medical check-up, depending on the risk of foot ulcer, according to international recommendations.

Control of glycemia in patients with type 2 diabetes mellitus and ischemic stroke

Abstract

The article deals with the results of clinical experimental studies, presented in literary sources, as well as observing their own approaches to the control of glycemia in patients with diabetes mellitus type 2 associated with stroke. Stroke is one of the main causes of death in patients with diabetes mellitus and is weighted by cerebral accident. Hyperglycemia is a proven trigger factor and the correction of hyperglycemia should be carried out at all stages of a stroke. In case of peracute period the hypoglycemic intervention is recommended when hyperglycemia above 10 mmol/l and we use the intravenous insulin infusion to maintain the range between 7.8-10.0 mmol/l. On the stabilization of condition of a patient we can observe the timely transition to antihyperglycemic therapy, to adequate diabetes course. Target range of glycemic fluctuations is 6.0-10.0 mmol/l in most cases. Taking into account the importance of preventing hypoglycemia, the application of metformin, dipeptidyl peptidase 4 inhibitors and insulin analogs with regular self-monitoring of glycemia is specially recommended.

Iodine deficiency: current status

Abstract

One of the priorities of health care in most countries is the prevention of iodine deficiency disorders (IDD). About two billion people live in areas of natural iodine deficiency. It is known that insufficient intake of iodine in utero and in early childhood is the greatest danger. Changes caused by iodine deficiency in these periods of life manifest in irreversible defects of intellectual and physical development. However, the entire spectrum of IDD is wide and extends from reproductive disorders to specific thyroid diseases.According to the World Health Organization, and (UNICEF) in 96 countries with natural iodine deficiency this problem is already solved due to laws and regulations on mandatory salt iodization, and only 13 countries do not have necessary laws, including Russia. It is proved that the Russian population continues to live in conditions of uncompensated iodine deficiencyIn this connection it is extremely relevant to conduct the mass and individual iodine prophylaxis using preparations containing a physiological dose of iodine (such as Iodomarin®) in high-risk groups.

Obesity (insulin resistance) and infertility are two sides of the same coin: pathogenetic interaction and opportunities of modern drug therapy

Abstract

The review article deals with the latest scientific data and with results of our own clinical studies and have shown the issues on pathogenetic links of obesity, insulin resistance and disorders of male reproductive function. All analyzed problems are extremely important andrological trend today associated with the continuing growth of obesity and insulin resistance frequency as well the growth of infertility among male population, which, of course, show the close pathogenetic links between them. Obesity and insulin resistance are system factors of sperm oxidative stress regardless of other (including urological) possible causes of male infertility; and clinical experience has shown that without timely diagnosis and pathogenetic drug therapy the reproductive function improvement in infertile males should not be expected. On the other hand, there are effective and safe drugs for reproductively obesity and insulin resistance correction in the arsenal of endocrinologists today and this treatment can be recommended for wider application in andrological routine practice. Solution of the problem concerning the drug therapy of male infertility in patients with metabolic disorders is possible only on the basis of an interdisciplinary approach, which requires inform practicing andrologists in detail about the obesity and insulin resistance mechanisms of affecting the spermatogenesis and modern pathogenetic approaches to eliminate them in men with infertility.

Non-alcoholic fatty liver disease (NAFLD) is an obesity-related disease associated especially with abdominal obesity, and various metabolic markers of cardiovascular disease and diabetes mellitus type 2 risk and all these parameters can affect the incidence of disease, prognosis and longevity of people with obesity. The article deals with the case study of observation and treatment of the patient with obesity and NAFLD, associated with a characteristic complex of metabolic disorders. We have shown that combined therapy, including the modification of lifestyle and application of metformin in people with obesity and NAFLD, is safe and effective correction of metabolic disorders.

The prospects of using the pharmacological effects of taurine in the treatment of patients with diabetes mellitus type 2

Abstract

The development of diabetes mellitus (DM) affects liver, disrupts the protein metabolism, amino acids, fats and other substances in hepatocytes, and predisposes to the non-alcoholic fatty liver disease. The treatment using oral hypoglycemic drugs can also make negative effect on the functional state of the liver. In some cases the combined drug treatment is prescribed, because of low efficacy of monotherapy and/or the presence of a patient with several diseases, particularly concerning the older people. Polypharmacy is one of the factors increasing the hepatotoxicity of drugs.Thus, not only the etiology and pathogenesis of the DM but also the application of complex long-term multi-component drug therapy requires the early hepatoprotectors application to prevent significant functional disorders of the liver in patients with DM. The drugs possessing metabolic type of action are advantageous now. The literature and experimental studies data let us suggest the reasonability of the complex therapy application using taurine - the sulfurcontaining amino acids, in patients with DM, which possess the hepatoprotective, and hypoglycemic effect.
